Need a Patient Portal username and password?

You will be given a username and password at the time you schedule an appointment. If you did not receive a username and password when scheduling, please call your doctor’s office using the phone numbers listed on this page, and we will provide one.

Forgotten your Patient Portal username or password?

Please call your doctor’s office using the phone numbers on this page during normal weekday office hours and ask for our staff to reset your user name and password. Given that your password is secure and known only to you, we cannot tell you what your password is, but we can provide a temporary reset password that will allow you to log in and set a new secure password.
