As a specialist clinic, Northwest Urology, LLC may perform a diagnostic procedure or collect a sample for labs, pathology, or genetic testing.Patients may still receive a separatebillfor in-house labs, pathology, genetic testing, diagnostic procedures, or additional services ordered during their appointment.These bills are non-negotiable.

• You may receive a separate bill from Northwest Urology for diagnostic procedures, lab tests or for the preparation on any samples collected for lab, pathology, or genetic testing.
• Any bodily fluids or tissues collected by Northwest Urology, LLC will be sent to an unaffiliated lab, and you will receive a separate bill from them for test and interpretation.
• Your insurance may not cover genetic testing.
• Your insurance may not cover the facility who test and interprets the genetic testing samples.
• Please contact your insurance if you have questions about your benefits and coverage.
