Our Financial Policy

At Northwest Urology, we are committed to delivering high-quality, personalized care at a fair and affordable cost. Our financial policies are designed to ensure transparency and eliminate surprises—providing you with a clear understanding of your financial responsibilities, including co-pays, co-insurance, deductibles, and any outstanding balances not covered by insurance or for self-pay patients.

Insurance Billing

You can view a list of commonly accepted insurances on this page. As a courtesy, we are happy to bill any insurance plan with which we are contracted. Please remember to bring your insurance card to your office visit so we can make a copy for our records.

If you have insurance and expect your carrier to cover some or all of your medical services, please note that your insurance contract is an agreement between you and your insurance provider. While we will assist with billing, you are ultimately responsible for all charges incurred, regardless of insurance coverage.

It’s a common misconception that medical insurance will automatically cover all services. However, coverage and reimbursement depend on your specific insurance plan and the policies of your insurance provider. Not all services we provide are covered by every plan; those services that the insurance determined to not be covered will be your responsibility. In order for us to submit claims on your behalf, we must have complete and accurate insurance information on file.

Co-payments, Co-insurance, and Deductibles

We require insurance co-payments, co-insurance, and deductibles to be paid at the time of service. We will bill you for any remaining balance that your insurance does not pay and that is required by your insurance.

Self-Pay

If you do not have insurance and are paying out-of-pocket, we offer a 20% discount on office visits and select in-office procedures. Payment is required at the time of service. If any additional charges apply, you will receive a bill for the remaining balance.

Payment Methods

For payment, we accept checks, credit cards, HSA cards, FSA cards and cashier’s check or money order. We do not accept cash in the office.

For most services we do offer a Card on File option, for any deductible or coinsurance amount, if applicable, at the time of service. The remaining balance will be billed to your credit or debit card once your claim is processed by your insurance. The card on file option can’t be used for the following services: circumcision, urolift, rezum, injections, space oar, vasectomy, vasectomy reversal, infertility, co-pay, or any self-pay appointments.
