Our Financial Policy

At Northwest Urology, we’re committed to providing the best in individualized diagnosis at treatment at a reasonable and affordable cost. Our financial policies have been developed in an effort to remove any surprises and provide you with a clear understanding of your account, including your financial responsibilities with regard to co-pays and co-insurance amounts, deductibles, and any balances not paid by your insurance or employer plan.

Insurance Billing

See our listing on this page for common plans we accept. As a courtesy, we are happy to bill any third-party insurance plan with which we’re contracted. Please bring your insurance card to your first office visit so we can make a copy of it and add your insurance information to our records. If you have insurance and you are expecting your carrier to pay for some or all of your services, we would like to remind you that your medical insurance contract is between you and your insurance company. Therefore, you are responsible for the charges incurred, regardless of insurance coverage.

Many people believe that if they have medical insurance, the insurance company will automatically pay for their services. This is not always the case. Whether your insurance company will pay for services and how much they pay depends on the coverage you have purchased and the insurance company’s own medical policies. Of course, in order for us to bill your insurance, you must supply us full information about your coverage.

As a rule, we can no longer bill 3rd insurances.

Co-payments

We require insurance co-payments and co-insurances to be paid at the time of service. We will bill you for any remaining balance that your insurance does not pay and that is required by your insurance. Your portion of any elective procedure must be paid prior to receiving services. Payment arrangements cannot be made for elective procedures.

Self-Pay and Payment Plans

If you do not have insurance and are paying for services yourself, we offer a 20% discount for office visits and some in-office procedures. However, payment in full is required at the time of service.

A financial payment plan may be offered if you require more time to pay for services. Please ask to speak with our staff in the billing office in advance at (971) 244-0798 to arrange for a plan that works for you.

Payment Methods

For payment, we accept cash, checks, Visa, MasterCard, Discover, and cashier’s check or money order. We do not accept American Express.

If you have insurance, you can run your credit/debit card through our Easy Pay program. Only the copay will be due at the time of your visit. Once your insurance company processes your claim, we will run your card for the exact amount due.
